This volume contains the proceedings of the International Symposium on Embryo Implantation: Molecular, Cellular and Clinical Aspects, held from October 3-6, 1997 in Newport Beach, California. Internationally recognized experts discuss the development and future of human in vitro fertilization and embryo implantation, the cellular aspects of implantation, hormonal regulation, molecular markers of receptivity, trophoblast factors, primate models and animal studies, and transcriptional regulation of maternal-fetal recognition.

This book provides an overview of the current knowledge of physiological and pathological immune reactions which favor or disturb fertility. The regulation of embryo implantation and trophoblast invasion through cytokines, growth factors and other pregnancy-related immunoregulatory molecules is described in detail. Autoimmunity and allergy are presented as possible causes of reproductive failure.

"Embryo implantation is a complex process, requiring the attachment of an activated blastocyst to a receptive endometrium and precisely controlled trophoblast invasion into the maternal decidua and spiral arterioles. The work included in this thesis aimed to identify key players in the maternal-fetal communication that occurs during embryo implantation. Chemokines are well known for roles in the regulation of leukocyte trafficking and extravasation. A previous study ... revealed ... [a possible role] for chemokines in the human endometrium outside of leukocyte recruitment. Chapters 2 and 3 of the thesis ... set out to identify roles for chemokines in embryo implantation. ... The aim of work described in chapter 3 was to elucidate mechanisms by which chemokines may stimulate trophoblast migration. ... In chapter 4, the aim was to screen human uterine lavages to identify proteins secreted into the uterine lumen, likely to be important for successful pregnancy. ... this thesis examines endometrial proteins and their roles during embryo implantation. Idenification of key molecular mediators at the maternal-fetal interface will provide better understanding of the complex cross talk essential for successful pregnancy and may provide novel targets for therapeutic strategies for the treatment of infertility."--Abstract, leaves vi-vii.

Novel topics compiled in this one-of-a-kind volume provide tantalizing hints for clinical application through future translational research. Only recently have we come to ask what role the male might play in pregnancy loss and embryo implantation failure, beyond contributing an abnormal set of paternal chromosomes at fertilization. This volume explores the recent evidence that these conditions might also be related to the transmission of previously unrecognized chromosomal micro-deletions and duplications, or via the epigenetic dysregulation of early embryonic gene function by spermatozoal microRNAs or alterations in sperm DNA methylation patterning, or by DNA damage secondary to either reactive oxygen species or environmental toxicants. In addition, the composition of seminal plasma has been found to be highly complex, containing many factors that play important roles in altering the uterine environment and the female immune system permissive of embryo implantation and trophectoderm outgrowth leading to successful pregnancy. Coverage includes an examination of both spermatozoa and seminal fluid component factors.

Implantation of the embryo into the maternal endometrium represents a crucial step in the reproductive process. It results from a cascade of finely tuned events. Despite recent progress in the field, many points still require clarification. This special issue offers a broad coverage of the latest findings. New concepts and applications are presented which are based on data strongly suggesting that both embryo implantation and uterine receptivity are controlled, primarily, though not exclusively, by locally acting growth factors and cytokines. Different processes are also studied, such as the MMPs expression regulating the trophoblast invasion, or the possible secretion of sHLA-G molecules by human embryos and its correlation to a successful pregnancy. Further points under discussion are the chromosomal imbalance leading to embryonic mortality, and the potential of the baboon model to study embryo implantation. Finally, a comparative survey is given of the general mechanical, endocrinological and cellular aspects involved in implantation in mammals. Clinicians as well as basic researchers will find this special issue to be a valuable source of new insights.

Interest in mechanisms of embryo implantation is increasing, particularly with the realization that failure of implantation after in vitro fertilization and embryo transfer places significant limits on the success of treatment. In addition, there is a need to provide hypotheses, and ultimately mechanisms, for the high rates of embryonic loss in women in the population at large. Traditionally, implantation research has concentrated on genetics and endocrinology without providing many therapeutic benefits. A new era is now beginning with the application of modem cellular and molecular approaches to the investigation of the relationship between trophoblast and endometrium. At the same time, older data can be reevaluated in the light of current research into cell cell and cell-matrix interactions. The feeling that new avenues of research are open was apparent when an international group of scientists came together at a workshop on "The Cell Biology of Trophoblast Invasion In Vivo and In Vitro" held during the XXIV Annual Meeting of the Cell, Tissue and Organ Culture Study Group (C.T.O.C.) at Heidelberg in 1986. What was unusual about this Conference was the interdisciplinary dialogue between implantation researchers and tumor biologists, highlighting aspects common to invasion of trophoblast and tumor cells.
